Legal Requirements Needed To Purchase Gaming Equipment

All paperwork requested must be in Worldwide Gaming
possession before your work order will commence.

Expect two to three week lead-times to complete work orders during busy times.

Where applicable, submit copy of Current Gaming License.

Where applicable, submit copy of Dept. of Justice Letter, Johnson Act 15 USC 1171.

Signed Worldwide Gaming Sales Invoice in agreement with your order from authorized personnel.

Signed Sales Contract Agreement from authorized personnel.

Signed Gaming Equipment Annual Statement from authorized personnel.

A 50% deposit has been received. Confirmed bank wire or company check issued.

 

Purchaser must live in a legal state where slot machines are not prohibited.

 

Blank Square Means Paperwork Required / Marked Square Means: Completed or Not Required.

Requirements Needed Before Gaming Equipment Will Be Released

*All paperwork must be in Worldwide Gaming
possession before any equipment will be released.
These rules will have- NO EXCEPTIONS.

Payment in FULL has been received in accordance with Worldwide Gaming Sales Invoice.

Bill of Lading stating ship to address declaring a legal jurisdiction.

Signed Freight Forwarder Commercial Invoice, (when shipping out of US).

Signed Shippers Export Declaration Form #7525-V, (when shipping out of US).
